Abstract
Lower extremity reconstruction presents the plastic surgeon with complex problems of not only form, but also of function. Regardless of the etiology of the lower extremity defectóbe it secondary to trauma, infection, vascular insufficiency, or tumoróthe extent of the tissue deficit often dictates a shift in traditional thinking about progression along a reconstructive ladder. When conceptualizing a reconstruction, use of the reconstructive ladder, with a stepwise advancement from more simple to increasingly complex reconstructive options, forces the plastic surgeon to evaluate and justify the added complexity and theoretical added risks at each successive rung on the ladder. In lower extremity reconstruction, issues of limb salvage, expediency of rehabilitation, extensiveness of defect, and location of defect form clear indications for moving directly to the highest rung of the ladder: microvascular free-tissue transfer.....
